The Chesapeake ADHD Center of Maryland office staff is generally available 9-6 every weekday. At other hours, leave a message in Mailbox # 10 and we will return your call to answer your questions or to schedule an appointment. Forms to complete after scheduling your first appointment: Once you have scheduled your appointment you will need to complete and FAX the Client Information Sheet and the Client Informed Consent Statement in order to confirm your appointment. You must FAX these forms to us. We reserve the right to cancel your appointment if we do not receive these forms. Please complete and FAX the appropriate intake form as well. The more information we have in advance of your initial appointment, the more we can accomplish in that initial session. In addition to questions asked on these forms, please feel free to write down any other information you feel will be helpful to us in understanding the problems for which you are seeking our consultation. If you prefer, you may complete the Intake forms at the Center at the time of your first appointment. If you plan on completing them at the center, please be sure to arrive at least 20 minutes in advance of your appointment time. In any case, the Client Information Sheet and Client Informed Consent Statement must be sent to us in order to keep your appointment.
In order to discuss your health information with anyone else (e.g. family member or physician), please fill out the Consent for Exchange of Information form as well. Parents please note that your child, 18 years or older, must sign this form in order for us to communicate with you. The Change of Payment Card Form is necessary if you wish to change the card we keep on file and/or pay with a different card for one appointment only. We cannot process a charge without your signature therefore this form must be filled out and signed by the card holder. What will happen during your first appointment Children and Adolescents Depending upon the age of the child we may see the parents alone, the child alone, or may divide the first assessment session between parents and child. Your first appointment will typically be a 90 minute clinical interview, during which we will ask you many questions about your current concerns, as well as about your child’s developmental, academic and psychological history. We will also ask about family history as it is pertinent to your child. Completing the intake questionnaire and sending it to us in advance of your initial appointment will facilitate the assessment process, allowing us to develop recommendations and begin treatment with your child right away. The first several sessions may focus on information gathering and assessment, depending upon the individual circumstances of the child. Most often, the Chesapeake staff member you consult with will recommend a neuropsychological or psychoeducational assessment. In some cases, the staff member you consult with will also administer some or all of the tests. Further testing may not be necessary if your child has been recently tested. Adults The first consultation will be a 90-minute clinical interview in which we will ask you about your current concerns, your reason for scheduling a consultation, and your family, educational and employment history. In many cases it is necessary to continue this interview process for a second appointment, which would be scheduled at the time of your first visit. Often, we will ask you to complete questionnaires following your initial consultation. We will review your responses to these questionnaires and discuss them with you at your second appointment. Testing is not automatically part of the assessment process for ADHD in adults. Diagnosis of ADHD is typically made through an extended clinical interview and the completion of ADHD questionnaires. If we believe that testing is desirable, such as in the case of suspected learning disabilities (which very commonly co-occur with ADHD) or when there are questions about diagnosis or co-existing conditions, we will discuss that with you during your initial clinical interview. Clients coming from out-of-town Many people contact us from other parts of the U.S. or from abroad to request an evaluation at the Center. When a client travels from a distance, we typically schedule a minimum two-hour long clinical consultation and, if testing is scheduled, we may attempt to schedule testing for that day or the following day, depending upon arrival time and staff availability. Follow-up after testing or assessment In the event that is not convenient for a client to return to the Center to discuss test results, we will schedule a telephone consultation to discuss test results and recommendations and can provide follow-up services via telephone as well.
